Ok, this is mIRC's $address command. $address($nick,1) is what you'd see when you check someone's /whois information, and is used as the standard mask for the "ban" command. Level 2 and 3 seem to be the most popular masks to use for bans since they're not as restrictive as 4. Level 4, just as it looks, bans the ISP completely, something that's usually popular for akicks of obscure ISPs. I'm not overly familiar with 5-9, but they're the same as 1-4 just with the nickname included.
You can check anyone's addy using mIRC //say $address(nickname,number)
